• We understand that starting to research therapists can be overwhelming and confusing! We strive to simplify that process by offering guidance and education via phone before scheduling. When you give our office a call, our office administrator, Madi, will give you detailed information about how our practice works and gather information in order to determine which therapist is the best fit for you (whether that is within our group or outside of our group). If we determine we are not the right fit, we’ll make sure you leave with referrals that will hopefully meet your needs. Our goal is to help individuals and families connect with mental health support that is right for them.

  • BHD is a fee for service practice and does not contract with any insurance companies. Although we are not in-network with any insurance companies, we are able to provide necessary information for out-of-network reimbursement and many of our clients do use their insurance plans in this way. BHD also accepts HSA and FSA accounts. We strive to offer services at various price points through employment of various professionals (e.g., Psychologists, Psychiatrists, Counselors and trainees). Please contact us to receive information related to the specific fees and services you may be needing.

    The No Surprises Act took effect January 1, 2022 which seeks to eliminate surprise medical billing, especially for individuals not using insurance to cover medical services. This act requires health care practices to let you know up front the cost of services associated with your treatment plan. You have the right to request a Good Faith Estimate of services prior to treatment and the right to updated estimates as treatment needs may change.

    At BHD each service is billed individually and clients may discontinue services at any time. Prior to scheduling, you will be informed of the exact cost of each service and you and your provider will discuss the recommended frequency of sessions based on your needs.

    A Consent for Treatment Form

    This is a long form (we know, sorry!) that explains a variety of important issues such as the philosophy behind therapy services at BHD, fees, services, your rights as a patient, our rights as providers, limits of confidentiality and other helpful information.

    A Texas Notice Form

    This form goes over limits of confidentiality and HIPPA.

    A Telehealth Consent Form

    This form reviews the pros and cons of telehealth treatment and ensures your understanding before beginning this modality of treatment.

    An in-office COVID-19 Policy

    This form reviews the current BHD COVID-19 procedures and policies.

    A Demographics and Background Information Form

    This form is used to collect basic information about you as a patient, your background, and why you’re seeking therapy at this time.

    A Release of Authorization or Information Form (Optional)

    A licensed mental health professional is required by law to keep all of your information confidential according to HIPPA standards. Completing an ROI form is the only way providers are allowed to discuss your care with anyone else (e.g., your partner, your doctor, your former therapist, etc.). Although this form is optional, we strongly encourage you to complete it with the name of the referring medical provider to ensure continuity of care. If you were not referred to our practice by a medical provider, please feel free to complete this form with the name of anyone else you may want to involve in your care.

    A Credit Card Information Form

    You may include a credit card (we accept all major credit cards), HSA or FSA account here. You will have access and control of your credit card through the client portal and can therefore change your card at any time.

    A Baseline Depression and Anxiety Screen

    Almost all clients will receive some sort of baseline mood and anxiety screen to allow your provider to have a better idea of how to meet your needs. This initial screen also allows us to create specific treatment plans and track your progress.

  • We currently offer in-person or telehealth therapy options. In-office visits are compliant with all CDC guidelines . Telehealth is conducted via video chat in our secure HIPPA compliant platform.

  • Our practice schedules appointments Monday through Friday during normal business hours. At this time, we do not offer weekend appointments. For scheduling options and therapist availability, please contact our office.

  • We offer services for men, women, adults, adolescents and children of all ages. We have providers in the group who are specialized in infant-mental health, pediatrics, adult general mental health and health related concerns, women’s health, parenting (both motherhood and fatherhood), and couples.
