Get Started at BHD

We know you have many choices when it comes to mental wellness providers. We take this seriously and trust you'll find comfort in our intentional process. 

It can feel challenging to call a mental health provider for the first time. Bringing you on as a new patient is easier and more comfortable than you might think. We’re here to show you how we thread our extensive education, training and experience together with our approachable and thoughtful process.

To get started as a new patient, we'll guide you along three simple steps:

Step 1: Provider Matching

To begin, call our office at 214-383-8145 or email info@behavioralhealthdallas.com.

You'll hear from Madi, our Intake Coordinator and Office Manager. If you reach us after business hours, you'll hear back from her within 24 business hours. 

She'll walk you through becoming a new patient by reviewing relevant practice information, answering any of your questions, and scheduling your provider-matching appointment. During this 20-minute phone discussion, Madi will work with you to identify the most suitable provider that aligns with your clinical and individual needs. 

Step 2: Intake

Once you’ve scheduled your intake appointment, you’ll receive an email invitation to your secure patient portal.

Your patient portal allows you to complete intake paperwork (required before your first appointment), access telehealth appointments, and view your upcoming sessions.

  • Consent for Treatment Form

    This is a long form (we know, sorry!) that explains a variety of important issues, such as the philosophy behind therapy services at BHD, fees, services, your rights as a patient, our rights as providers, limits of confidentiality, and other helpful information.

    Texas Notice Form

    This form goes over the limits of confidentiality and HIPAA.

    Telehealth Consent Form

    This form reviews the pros and cons of telehealth treatment and ensures your understanding before beginning this type of treatment.

    Demographics and Background Information Form

    This form collects basic information about you as a patient, your background, and why you’re seeking therapy at this time.

    Release of Authorization or Information (ROI) Form (Optional)

    A licensed mental health professional is required by law to keep all of your information confidential according to HIPAA standards. Completing an ROI form is the only way providers are allowed to discuss your care with anyone else (e.g., your partner, your doctor, your former therapist, etc.). Although this form is optional, we strongly encourage you to complete it with the name of the referring medical provider to ensure continuity of care. If you were not referred to our practice by a medical provider, please feel free to complete this form with the name of anyone else you may want to be involved in your care. 

    Credit Card Information Form

    You may include a credit card (we accept all major credit cards), HSA, or FSA account here. You will have access and control of your credit card through the client portal and can, therefore, change your card at any time. 

    Baseline Depression and Anxiety Screen 

    As a new patient, you'll receive a baseline mood and anxiety screen to inform your provider of how best to meet your needs. This initial screen also allows us to create specific treatment plans and track your progress.

Step 3: Getting to Know your Therapist

What to expect at your first therapy session:

  • When you visit our office, you'll enter the waiting room and see a set of switches to notify your therapist of your arrival. After you flip the switch, your therapist will greet you in the waiting room and walk you back to their private office. All BHD offices are designed with your ease in mind.

    Our patients often share positive remarks on the soothing atmosphere from thoughtful artwork, airy and light-filled rooms, and comfortable seating.

  • If your first session is with telehealth (virtual), you'll log in to the patient portal and click on your session to join. Prior to each session you will receive email reminders including dates, times and portal information.

You'll attend your first session and get to know your therapist. During your first appointment, your provider will ask you lots of questions and answer any of yours. From there, your first 2-4 sessions are a mutual evaluation to ensure your provider is the right clinical and personality fit for your needs. You'll be evaluating your therapist to make sure you feel comfortable with them and they will be completing their evaluation to understand your preferences in therapy, defining the problem (diagnoses), and building an individualized treatment plan in which you’re confident.

After that, you'll be working through the treatment plan and it will feel more like what therapy will "feel" like on an ongoing basis"
